What we prescribe

Bioidentical hormone therapy, done thoughtfully

Every prescription is based on your labs, your symptoms, and your risk profile. We use FDA approved bioidentical formulations.

Estradiol
Bioidentical estrogen

Restores estradiol to physiologic levels to address hot flashes, sleep, mood, and vaginal and cognitive symptoms.

Progesterone
Bioidentical progesterone

Paired with estradiol for uterine protection and for its independent benefits on sleep, mood, and anxiety.

Testosterone
Yes, for women too

Low dose testosterone can restore libido, energy, mood, muscle mass, and cognitive clarity when levels are low.

Thyroid & adrenal
Full hormonal picture

Thyroid optimization (T4, T3, or NDT) and DHEA or adrenal support when your labs and symptoms warrant it.

What your hormones are doing

The hormone shifts behind the symptoms

Estrogen, progesterone, and other hormones fluctuate across your cycle and life. Understanding these patterns is what lets us build a plan that actually fits you.

Why this matters

Fixing hormones is bigger than feeling better

Hormones do more than manage symptoms. They protect your long term health across every major system. That is why we take the workup seriously, and why hormone care is one of the highest impact things you can do for your future self.

Estrogen
Brain · Heart · Bones

Beyond symptom relief, estrogen protects long term brain health, cardiovascular function, and bone density. Untreated estrogen loss accelerates osteoporosis and increases cardiovascular risk. Modern evidence also links appropriately timed estrogen therapy to reduced risk of cognitive decline.

Progesterone
Sleep · Mood · Brain

Beyond uterine protection, progesterone supports the calming GABA pathway in the brain, improving sleep quality, mood regulation, and anxiety. It also has neuroprotective and bone supporting properties.

Testosterone
Muscle · Libido · Cognition

Women's testosterone declines with age too. It supports libido, muscle mass and strength, cognitive clarity, mood, and metabolic health. Restoring testosterone can be the difference between managing symptoms and actually feeling like yourself.

Thyroid
Metabolism · Energy · Heart

Optimal thyroid function is required for metabolism, energy, weight regulation, mood, and cardiovascular health. Perimenopause commonly unmasks thyroid dysfunction that has been simmering for years.

Add-on testing when you need more answers

Some cases warrant a closer look at your specific hormone patterns. When clinically indicated, we offer these advanced options as add-ons.

MIRA Monitor
At-home hormone tracking

An FDA cleared at-home device that measures estrogen (E3G), LH, PdG (progesterone metabolite), and FSH from urine samples. Especially valuable in perimenopause: you can see exactly what your cycle is doing day by day and tie hormone shifts directly to your symptoms.

DUTCH Testing
Dried Urine Test for Comprehensive Hormones

A detailed look at how your body is metabolizing sex hormones and cortisol across the day. Helpful for identifying detox and metabolism issues, estrogen dominance patterns, and adrenal rhythm problems that a standard blood panel cannot see.

Adrenal Testing
4-point salivary cortisol

Measures your cortisol rhythm at four points across the day to evaluate HPA axis function. Helpful for chronic stress, unexplained fatigue, sleep issues that do not resolve with hormone therapy alone, and burnout patterns.

Safety & evidence

Rooted in the research, personalized to you

The Women's Health Initiative changed the conversation about hormone therapy, and decades of subsequent research have refined what we understand. Modern evidence supports hormone therapy as safe and effective for most women when started at the right time and monitored properly.

Our protocols follow evidence based clinical guidelines. Every prescription includes appropriate safety monitoring for your specific case.

Timing mattersStarting hormone therapy in the perimenopausal or early menopausal window is associated with the best safety profile.
Personalized, not protocolizedYour labs, history, and risk factors determine your plan. We do not use one size fits all dosing.
Routine safety monitoringRegular re-checks, symptom tracking, and appropriate labs (like CBC when you are on testosterone) are standard.
Frequently asked

What patients ask before starting

What is the difference between perimenopause and menopause?+

Perimenopause is the transition period leading up to menopause, when hormones begin to fluctuate. It can start in your late 30s or 40s and last several years. Menopause is officially the point when you have gone 12 consecutive months without a period. Symptoms can be similar during both phases, and treatment is often needed years before you technically reach menopause.

Is hormone replacement therapy safe?+

For most women, current evidence supports that hormone therapy is safe and effective when started at appropriate times and monitored properly. The concerns raised by the original Women's Health Initiative study have been refined by decades of subsequent research. At NEOS, hormone therapy is always personalized based on your individual health history, labs, and risk factors, with ongoing safety monitoring.

What are bioidentical hormones?+

Bioidentical hormones have the exact molecular structure as the hormones your body naturally produces. NEOS prescribes FDA approved bioidentical estradiol and progesterone rather than synthetic versions, which allows for more precise dosing and better tolerability for most patients.

Do women need testosterone?+

Yes. Women produce testosterone throughout their lives and it plays an important role in libido, mood, energy, muscle mass, and cognitive function. When testosterone levels decline, women can experience symptoms that estrogen and progesterone alone do not resolve. Low dose testosterone can be a safe and effective part of a complete menopause protocol.

When is the right time to start hormone therapy?+

You do not have to wait until menopause. Perimenopausal symptoms can begin in your late 30s and 40s and often benefit from treatment years before your final period. Current evidence supports starting hormone therapy in the perimenopausal or early menopausal window, generally within 10 years of your final period, for the best safety and benefit profile.

How long can I stay on hormone therapy?+

This is decided individually based on your symptoms, risk factors, and goals. Many women benefit from long term hormone therapy well beyond the traditional five year window that older guidelines suggested. NEOS reassesses your protocol regularly and adjusts as your needs change.

What labs do you run before starting?+

A comprehensive baseline that includes CBC, CMP, full thyroid panel (TSH, free T3, free T4, thyroid antibodies), estradiol, progesterone, testosterone (total and free), DHEA-S, cortisol, FSH, LH, metabolic markers such as fasting insulin, HbA1c, and a full lipid panel, plus micronutrient testing to identify deficiencies that can drive fatigue, mood, and hormone imbalance.

Do you offer advanced testing like DUTCH, MIRA, or adrenal panels?+

Yes, as add-on testing when clinically indicated. MIRA monitor is an at-home device that tracks estrogen (E3G), LH, PdG (progesterone metabolite), and FSH from urine samples. It is especially useful in perimenopause because you can see exactly what your cycle is doing day by day and tie hormone shifts directly to your symptoms. DUTCH testing (Dried Urine Test for Comprehensive Hormones) shows how your body is metabolizing sex hormones and cortisol across the day, which is helpful for detox patterns, estrogen dominance, and adrenal rhythm issues that blood work cannot see. Four-point salivary cortisol testing evaluates HPA axis function and is helpful for chronic stress, unexplained fatigue, and burnout patterns.
